1. How long have you had Dupuytren's contracture?
2. How old were you when your DC first appeared?
3. Are one or two hands involved?
4. How many fingers are involved?
5. Do you think your DC started due to a family history (genetic tendency) for Dupuytren's contracture; other members of your family have DC?
6. Do you think your DC started because of single severe injury or surgery of the upper extremity?
7. Do you think your DC started because of repeated and prolonged hand stress and over-use?
8. For the men - do you also have Peyronie's disease?
9. Do you currently have only a lump(s) or nodule(s) on your palm, and no cord(s)?
10. Do you currently have both a lump(s) on your palm and a cord(s) going into your finger(s)?
11. Since your DC started, has the palm lump(s) been constant; has the palm lump always been there and never got smaller or thinner?
12. Since your DC started, has the palm lump(s) been variable; did it ever change or reverse itself; sometimes smaller / better and sometimes bigger / worse? Did the palm lump or nodule look like it was going away, but then it would come back again?
13. Soon after your DC started did the palm lump(s) respond favorably to something you did (like stretching, heat, massage or lotions) so that it changed or improved in some way, that it did not respond to later as it got larger and harder?
14. If your DC got better and worse after it started, how many times did the palm lump(s) go through this back and forth cycle, and over what span of time did this occur? Example: 2 times in 1 year; 6 times in 10 years.
15. If your DC got better and worse after it started, how long was the palm lump(s) better when it happened; when it got better or reduced, how long did that last? (Examples: It went away for a month or two and then came back. My palm lump was almost gone for about nine months but then it got worse again.)
16. If your DC got better and worse after it started, for how long did the palm lump(s) return; when it got worse how long did it last? (Examples: Every time it returned again it lasted about six months. My palm lump came back and got even thicker for maybe 6-8 weeks each time.)
17. If your DC got better and worse after it started, did the palm lump ever completely disappear or clear up before it returned again?
18. If your DC got better and worse after it started, do you think it changed because of something you did or something that happened to you to cause those changes? What do you think caused it to change so much, please?
